A beautiful smile should never come at the expense of healthy tooth structure, function, or the biology that supports it.
The A+BCDE™ (Airway Positive Biomimetic Comprehensive Dental Esthetics) Protocol is Dr. Keerti Sahasrabudhe’s comprehensive approach to smile design. Developed through decades of advanced education, clinical experience, and interdisciplinary training, it integrates airway-conscious treatment planning, biomimetic dentistry, comprehensive diagnosis, and cosmetic excellence into a single philosophy of care.
Dr. Keerti is one of fewer than 100 Fellows of the American Academy of Cosmetic Dentistry worldwide and an Accredited member of the Academy of Biomimetic Dentistry. Since 2015, she has incorporated airway evaluation into every comprehensive examination after being influenced by pioneers in airway-focused orthodontics.
Rather than asking, “How do we make the teeth look better?” the A+BCDE™ Protocol begins with a more important question:
A real patient example to understand the A+BCDE™ Protocol for smile makeover
This patient, a young dentist herself, sought out Dr. Keerti specifically for this comprehensive approach rather than pursuing conventional veneers.
Evaluation revealed a narrow upper arch, creating dark buccal corridors, a constricted smile, and tooth proportions that appeared long and narrow. Her width-to-length ratio measured approximately 69%, whereas proportions closer to 72-75% generally create a fuller, more balanced appearance.
A conventional veneer approach would have widened the teeth by reducing healthy enamel.
Instead, Dr. Keerti first expanded the dental arches. Expansion naturally created the spaces in between teeth needed to improve tooth proportions with additive porcelain veneers. So, not reductive treatment that cuts into teeth to create room for porcelain but in fact an additive, tooth-preserving solution!
The final treatment required only four minimally prepared veneers in the esthetic zone. The canines were left completely untouched because they already possessed excellent shape, size, shade, and proportion. Necessary restorative treatment was completed on the posterior teeth to replace failing dentistry.
By addressing the underlying problem first, the final treatment was more conservative, more biologically respectful, and produced a broader, more harmonious smile.
This case went on to receive Seattle Study Club’s national Best Case Wins award in recognition of its comprehensive diagnosis, interdisciplinary planning, conservative execution, and exceptional esthetic outcome.
The Five Principles of the A+BCDE™ Protocol
More Than Cosmetic Dentistry
The A+BCDE™ Protocol reflects Dr. Keerti’s philosophy that exceptional esthetic dentistry begins with understanding the whole patient. By addressing underlying problems first and preserving what nature has already created, treatment becomes more conservative, more predictable, and ultimately more beautiful.
Beauty from the Inside Out.
